Swim & Gym - Important Forms

IMPORTANT FORMS

At Swim & Gym Sports Camp, we use state-certified forms to ensure each child's safety and well being. When enrolling please complete each of the forms below and return them with your payment at least two weeks before the start of your child’s first session of camp. If you are registering on line, after you fill out the forms and submit them, please print them off and bring a copy with you the first day of camp.
