Summer Camp Registration
Thank you for your interest in Swim&Gym All Sports Camp, LLC! Here's some useful information about camp registration.

Swim&Gym will begin accepting applications January 2nd. We cannot accept applications by phone.
Please fill out one application per camper. You can download a summer camp application from the site.
We need a parent signature to process the registration.
If you do not register online, please stop by our office, or mail your application and weekly fees to: SwimWest, 1001 Deming Way, Madison, WI 53717
Please make checks payable to Swim&Gym. Only one check is needed for multiple campers or fees.
The full amount paid will be refunded if a camper cannot be placed in the session of his/her choice. The balance of fees paid will be refunded if a camper cannot attend due to injury or illness with notification prior to the start of camp. Individual missed days due to injury or illness once your session has begun will not be refunded. In all other instances, no refund of camp fees will be given unless notice of cancellation is given to camp at least two weeks prior to the beginning of the camper's session. If a camper leaves early due to disciplinary reasons, or parent request, no refund will be given.
If you pay the camper fee in full by March 31st, you will receive a $25 discount per family.

BEFORE submitting your camp application online, please print all forms for your records and print a copy to send to SwimWest.
Important forms can be down-loaded and/or picked-up from SwimWest.
After your application is received you will receive a confirmation and welcome email, and a registration receipt.

After you've downloaded or picked-up your informational packet, fill out the forms completely. Forms must be submitted to SwimWest no later than two weeks before camp attendance.
Swim&Gym will honor up to two friend requests. Please indicate requests for friends to be in the same group on the registration form. (Please note that friend requests will only be honored if both campers are in the same age group and program, and independently register for the same session before it fills.)
